A nonrefundable enrollment fee of $150, when completing the Student Registration Application form below, will be needed to secure your child's spot in the Early Childhood Program based on availability. Monthly tuition is to be paid in 10 equal installments beginning August 1, 2025. The final automatic charge to the card on file will occur on May 1, 2026. If your family decides to leave the program early, then you will not be charged on your next payment date and your child's last day will end the day before the next payment is scheduled. Payments will not be prorated for days your child is not there. A Cancelation Form will need to be signed and submitted 7 days before your child's last day. All forms will need to be filled out to be accepted into The House of Children Early Childhood Program at Maria Montessori Academy.

We pride ourselves on having a small teacher-to-student ratio. With that, we only have 21 spots available for the 2025-2026 school year.

Required Forms to Complete Registration

To complete your child's registration to enter the Early Childhood Program at Maria Montessori for the 2025-2026 school year, all documents to the right need to be read, checked off, and signed. Then all 5 forms below need to be filled out and submitted. Once completed and payment submitted, you will receive a confirmation email with your receipt. Please contact us if you are having any trouble filling out forms or payment.

Student Questions

By signing this form, you indicate that you have read and agree to the following terms;


• Preschool students are only students of the The HOUSE of Children (THOC) and not enrolled as students of Maria Montessori Academy (MMA). Students attend MMA classes exclusively as authorized visitors.


• Preschool students are not eligible to receive special education services from MMA.


• Students will not receive any special preference for enrollment into MMA as a result of their participation in the preschool program.


• Preschool students and parents will be expected to follow all MMA policies and procedures to the same extent as students enrolled in MMA, which includes the Dress Code policy.


• MMA nor THOC provides transportation services to any students.


• Parents’ contractual relationship is with THOC and not with MMA. MMA administration has the authority to determine disciplinary consequences for preschool students and, if determined to be in the School’s best interest, to exclude any preschool student from participating in the program.


• I agree for THOC to share with MMA confidential student and parent information, as MMA and THOC deem appropriate.

Send Required Forms

A Birth Certificate and Vaccination Records or a Vaccination Exemption Form from a physician will be needed before the school year starts. Please submit forms below.

Consent for Treatment

By signing this Consent for Treatment, I, as the parent/guardian of the child named above, hereby freely, knowingly and voluntarily give the staff (including employees or volunteers) permission and authority to:


(A) perform and provide first-aid treatment in the event my child suffers or experiences any minor physical injuries or incidents (e.g. cuts, bruises, nosebleeds, etc.); and 


(B) obtain assistance from emergency medical personnel in the event my child suffers or experiences any seizures, major illness or major physical injuries as determined by Ogden Valley Science Summer Camp staff.


If emergency medical personnel are summoned, a member of The HOUSE of Children staff will make reasonable efforts to contact me at the emergency telephone number provided below.  I further understand and agree that, once such emergency medical personnel have arrived, they will solely determine the extent of treatment necessary to properly treat my child’s condition or injury.


I further understand and agree that The HOUSE of Children staff cannot and will not provide my child with any medications whatsoever in the event he/she experiences or suffers any illness or injury.


I fully understand neither The HOUSE of Children nor The Householder Group, LLC, will provide medical insurance for any medical treatment that may be performed on my child, and that any costs directly indirectly arising out of or related to any such treatment will be at my sole expense and/or must be covered by any insurance coverage I may have obtained for my child.


By signing this Consent for Treatment, I hereby fully waive, release and discharge The HOUSE of Children and The Householder Group, LLC, and their respective owners, members, shareholders, officer, directors, employees, agents and volunteers (collectively, the “Released Parties”) from any and all rights, claims and actions whatsoever including, for example and without limitation, any costs, expenses or attorneys’ fees, which may directly or indirectly arise out of or result from any first-aid or medical treatment that my child may receive.  Furthermore, I agree to forever indemnify, defend and hold the Released Parties harmless from and against any and all claims, actions or costs whatsoever that may directly or indirectly arise out of or result from any injury or harm my child may experience or suffer as a result of any such first-aid or medical treatment.


I have read this Consent for Treatment in its entirety and fully understand its contents.  I agree that if any portion is held invalid, the remainder of this Consent for Treatment will continue in full legal force and effect.

Liability Release & Parental Consent

By signing this Liability Release and Parental Consent, I, as the parent/guardian of the child named above, hereby freely, knowingly and voluntarily give my child permission to participate in any activities that may be conducted as part of The HOUSE of Children. 


I fully recognize that my child’s participation in activities may pose various risks such as, for example, physical injury resulting from falling, insect bites, bee stings, etc.  I acknowledge that I have been advised of such risks to my child’s safety in relation to these activities, and I hereby consent to his/her participation in these activities with a full and complete awareness of the risks involved.  I acknowledge that my consent to my child’s participation in these activities is voluntary and informed.


By signing this Liability Release and Parental Consent, I also hereby fully waive, release and discharge The HOUSE of Children and The Householder Group LLC, and their respective owners, members, shareholders, officer, directors, employees, agents and volunteers (collectively, the “Released Parties”) from any and all rights, claims and actions whatsoever including, for example and without limitation, any costs, expenses or attorneys’ fees, which may directly or indirectly arise out of or result from my child’s participation in any activities or events that may be arranged, conducted and/or supervised by The HOUSE of Children or any other Released Party.  Furthermore, I agree to forever indemnify, defend and hold the Released Parties harmless from and against any and all claims, actions or costs whatsoever that may directly or indirectly arise out of or result from any injury or harm my child may experience or suffer during the course of his/her participation in any such activities.


I have read this Liability Release and Parental Consent in its entirety and fully understand its contents.  I agree that if any portion is held invalid, the remainder of this Liability Release and Parental Consent will continue in full legal force and effect.

Photo/Video Release

By signing this Photo/Video Release, I, as the parent/guardian of the child named above, acknowledge, understand and agree that my child may be occasionally videotaped and/or photographed while participating in The HOUSE of Children activities and events.

By checking any or all of the four box(es) immediately below, I hereby grant The HOUSE of Children and The Householder Group, LLC permission to use videos and/or photos of my child for the following purposes:

Potty Training Form

In order to attend The HOUSE of Children your child must be potty trained, we are in the facility and staff of Maria Montessori Academy with kindergarten age children.  Our staff is not equipped or trained to potty train students, thus it is in agreement that your child is potty trained before attending The HOUSE of Children.  We recognize there will be accidents on occasion, we ask that you have 1-2 change of clothes in the case of an accident.
